A peptide known online as the 'Wolverine' compound is being used by a rapidly growing number of American patients despite the fact that it cannot legally be sold as an approved drug in the United States, according to a new medical-records analysis. Doctors have documented its use for ailments ranging from torn tendons to gastrointestinal disorders, an unusual real-world portrait of a substance that has spread largely through social media and word of mouth rather than through a pharmacy counter.
BPC-157 is a synthetic version of a protein fragment first isolated from human gastric juice, and for decades it existed almost exclusively in laboratory studies on rats, where it appeared to speed the healing of torn muscles, tendons and stomach ulcers. It picked up its comic-book nickname because online sellers and biohackers claim it regenerates injured tissue the way the X-Men character's healing factor does, especially when combined with a second unapproved peptide called TB-500, a pairing widely marketed as the 'Wolverine stack.'
A Trend Hiding in Plain Sight
Because BPC-157 has no official U.S. drug code, it leaves almost no trace in the billing and prescription data researchers normally rely on to track medication use. To get around that, the health-data analytics firm nference combed through more than 15 million medical records, searching physicians' clinical notes for any mention that a patient was taking it. The search turned up 1,039 patients whose charts documented BPC-157 use, a figure the study's lead author, Venky Soundararajan, said likely understates real use — in his view, few physicians think to ask patients directly whether they're taking it.
Even that undercount showed a striking trend: the number of newly confirmed BPC-157 users rose 33-fold between 2020 and 2026. The people taking it have changed too. In 2020, just over half of new users were men; by this year that share had climbed to 70%. Average age fell from 55 to 49 over the same period, and among patients whose race was recorded, 96% were white.
Why Patients Are Reaching for an Unapproved Compound
Doctors recorded a specific reason for treatment in 644 of the cases. The most common were sports injuries, chronic pain and gastrointestinal complaints — conditions that broadly track the animal research suggesting BPC-157 might promote tissue repair and gut healing. Patients most often obtained the peptide through a compounding pharmacy or an unregulated online and gray-market seller, rather than through a standard retail prescription.
What happens to patients afterward is murkier. Doctors recorded a treatment response for only about a third of the group, and while some notes described symptom improvement, researchers said the data was too sparse to draw conclusions about effectiveness. When patients stopped taking BPC-157, it was usually on a physician's advice; a smaller number quit after adverse effects that included psychiatric symptoms, skin reactions and gastrointestinal problems.
A Peptide With a Powerful Backer in Washington
The rise of BPC-157 coincides with an unusually sympathetic hearing from federal regulators. On July 23, the FDA's Pharmacy Compounding Advisory Committee voted 8 to 6, with one abstention, to recommend adding BPC-157, alongside two related peptides, TB-500 and KPV, to the list of substances compounding pharmacies may legally prepare for patients with a prescription. The recommendation is advisory only; it does not amount to FDA approval and does not immediately change federal rules, which still require a formal rulemaking process that typically takes the better part of a year.
The vote came over the objections of the FDA's own career scientists, who told the committee they lacked sufficient evidence to judge whether the peptides were safe or effective. It also followed the appointment of several panel members with ties to the peptide industry since Health Secretary Robert F. Kennedy Jr., a self-described enthusiast of peptide therapies, took office. Industry analysts have estimated a legitimized market for BPC-157 and similar compounds could be worth $2 billion to $3 billion.
What Science Still Doesn't Know
The evidence base for BPC-157 remains almost entirely preclinical. Decades of published research, much of it from a single laboratory group in Croatia, has shown the peptide speeding wound and ulcer healing in rats, but only a handful of small human studies exist, including one that followed just a dozen patients who received injections for knee pain. Outside researchers who have reviewed the literature independently have cautioned that enthusiasm for BPC-157 has outpaced the science, and that its long-term effects in humans remain essentially unknown.
Because much of the compound sold today comes from unregulated suppliers rather than licensed pharmacies, medical-fraud researchers have also flagged contamination as a separate risk, noting that peptides bought outside the regulated supply chain have previously turned up with heavy-metal impurities and inconsistent dosing. For now, BPC-157 remains unapproved in the United States, its popularity rising largely on the strength of online testimonials and a health secretary's endorsement rather than the large human trials regulators typically require before a drug reaches pharmacy shelves.
